The application form arrived within one week of request. It included a form that you had to sign releasing data concerning your mental health, but the record search confined to State of Maine.
No Notarization of application needed.

Home State Permit (Plus Copies of ALL Permits held) REQUIRED
Fingerprints not needed
Proof of training REQUIRED
No references needed
Cost 60 dollars
Term 4 years
I sent the completed application on 18 August 99. My permit was received here on 5 October 99……just a little over a month.”
